Note: This version is setup to be printed on a duplex (both sides) printer with flip along short side. If you do not have a duplex printer, you will have to first print pages 1,3,5,7,9 - Then reverse order the pages - Turn then over - Put them back in the printer - Then print pages 2,4,6,8,10.

Once you have the print out, fold the paper in half to form the 5 1/2 by 8 1/2 booklet.
